Dental practitioners, medical practitioners and other practitioners and/or their practice staff, can refer to MOSS by contacting info@mobileoralsurgeryservices.com.au and outlining the following:
- Reason for referral
- The patient’s contact information including mobile number and email address
- If available, a copy of their most recent large jaw X-ray (OPG). Please include the date in which the large jaw X-ray (OPG) was taken
MOSS will then contact the patient to complete the online MOSS Patient Registration Form in order to be eligible for a consultation appointment.
MOSS also accepts referrals from patients (self-referrals) by completing the MOSS Patient Registration Form found here or scan the QR code below.
